Shivaya namaha, Shivaya namah Om

Transliteration:

shivāya namaha, shivāya namah oṃ

shivāya: "to Shiva," as in "Om, all glory to Shiva." Shiva is "The Auspicious One," the Supreme God, the Destroyer and Transformer, the Lord of the cosmic dance of all creation and destruction.

namah/namaha: (Same as "namo") To honor or solute. "All glory to..."

oṃ: The eternal source-sound-vibration of creation.
